PraxysEHS Launches the Human Factors Incident Analysis Tool
Guided software helps teams learn what the work demanded, not just what the worker did.
OHIO, October 12, 2026: PraxysEHS LLC, an Ohio-based workplace safety and health company, has launched the PraxysEHS Human Factors Incident Analysis Tool, a web-based coach that guides safety professionals, supervisors and business owners through incident and near-miss investigations. It is available now at praxysehs.com, with a free trial and no credit card needed.
Too many investigations still end with the same two words: “employee error.” The corrective action is retraining, and the same incident comes back a few months later. PraxysEHS starts from a different idea: people do what the work asks of them, in the conditions they have. To prevent the next incident, you have to understand those conditions.
The tool guides users through the analysis a seasoned investigator would run, looking at three sources side by side:
- Paper: what's documented, such as procedures, work instructions, training records and plans.
- People: what the workers involved say about how the job really gets done, including fatigue, time pressure, distractions and workarounds.
- Places: what the workplace itself shows, at the spot where the work happens.
Where those three don't line up, there's something to learn.
The approach grew out of field work. PraxysEHS collaborator Blaine J. Hoffmann, MS OSHM, developed Paper, People, Places while conducting VPP gap assessments for industrial clients and supporting the U.S. Air Force Research Laboratory as it pursued a partnership with OSHA. VPP onsite verification draws on document review, interviews and observation. Hoffmann found that the order of those sources matters, and that teams needed a repeatable way to gather the right information from each. PraxysEHS builds that sequence into every investigation.
The tool's key features:
- A guided path, not a blank form. Plain questions in a set order, from the first hour to fixes that stick, so first-time investigators always know what to do next.
- “I don't know yet” becomes a task. Every open question turns into an assignment with an owner and a due date, so nothing quietly disappears.
- Fixes that hold. Retraining alone won't close a finding. The tool pushes for fixes that change the work itself, then has someone check that they're working.
- Patterns you can see. Findings are labeled by cause in plain language, so the same causes stand out when they keep showing up.
- Every near miss, no extra charge. Investigations are never capped, so teams can look into small events and concerns before someone gets hurt.
“Most people asked to run an investigation have never been taught how. They get a form and a deadline, and the easiest box to check is ‘employee error.’ We built PraxysEHS to be the coach in the room, so anyone can find out what the work really demanded and fix that,” said the PraxysEHS team.
“In VPP gap assessments, I kept seeing teams with the right three sources (documents, interviews and observation) but no repeatable way to use them. Start with the paper and you know what was expected. Talk with the people and you learn how the job really gets done. Go to the place and you see the conditions for yourself. Do it in that order, the same way every time, and the gaps show up. PraxysEHS puts that process in the hands of anyone running an investigation, so the conversation moves from who messed up to what the work demanded,” said Blaine J. Hoffmann, MS OSHM, producer and host of The SafetyPro Podcast and a PraxysEHS collaborator.
PraxysEHS is built for safety managers at a single site, EHS directors across many sites, operations managers, owners without safety staff, consultants managing client cases, and HR managers at small businesses. It fits programs that rely on worker involvement and thorough incident investigation, including VPP sites.
